In 2003, there were 2293 reported accidents, 1391 mandatorily reported incidents, 739 voluntarily reported occurrences, and 172 fatalities.
In 2004, there were 2220 reported accidents, 1489 mandatorily reported incidents, 781 voluntarily reported occurrences, and 186 fatalities.
In 2005, there were 2276 reported accidents, 1373 mandatorily reported incidents, 704 voluntarily reported occurrences, and 188 fatalities.
In 2006, there were 2155 reported accidents, 1326 mandatorily reported incidents, 710 voluntarily reported occurrences, and 167 fatalities.
In 2007, there were 2109 reported accidents, 1410 mandatorily reported incidents, 921 voluntarily reported occurrences, and 153 fatalities.
In 2008, there were 1865 reported accidents, 1459 mandatorily reported incidents, 742 voluntarily reported occurrences, and 161 fatalities.

Figure 2 - Investigations Started, in Process, and Completed
In 2003-2004, there were 76 investigations started, 142 investigations in process, and 73 investigations completed.
In 2004-2005, there were 75 investigations started, 102 investigations in process, and 115 investigations completed.
In 2005-2006, there were 78 investigations started, 105 investigations in process, and 75 investigations completed.
In 2006-2007, there were 69 investigations started, 120 investigations in process, and 54 investigations completed.
In 2007-2008, there were 70 investigations started, 108 investigations in process, and 82 investigations completed.
In 2008-2009, there were 61 investigations started, 81 investigations in process, and 88 investigations completed.

From 1999 to 2008, 57 % of the responses to recommendations were rated as Fully Satisfactory.

In 2003, there were 547 marine reported accidents, 224 mandatorily reported incidents, 70 voluntarily reported occurrences, and 17 fatalities.
In 2004, there were 492 marine reported accidents, 248 mandatorily reported incidents, 66 voluntarily reported occurrences, and 28 fatalities.
In 2005, there were 489 marine reported accidents, 227 mandatorily reported incidents, 121 voluntarily reported occurrences, and 20 fatalities.
In 2006, there were 472 marine reported accidents, 216 mandatorily reported incidents, 114 voluntarily reported occurrences, and 18 fatalities.
In 2007, there were 456 marine reported accidents, 228 mandatorily reported incidents, 128 voluntarily reported occurrences, and 14 fatalities.
In 2008, there were 418 marine reported accidents, 247 mandatorily reported incidents, 90 voluntarily reported occurrences, and 24 fatalities.

No pipeline fatality has been reported since 1988.
